                   I,                          Mia, S/o-                  Mia of sonamura Hospital road, w/No-6,PS.                           Dist.Sepahijala Tripura, aged 36 yrs, by Cast Muslim, by profession-           , citizen of India, do hearby solemnly affirm and declare as under:-

1)                  That my son’s namely Pradip Debbarma was born on 23/3/18 A/D at our own residence .Mother name is Ratna Debi Debbarma.

 2)   That I could not get entry his name and Date of Birth in Birth Register of our panchayet in due time due to mistake

3)    That I am swearing this affidavit for obtaining his BIRTH CERTIFICATE from the concerning authority
