  We  a),-------- Chowdhury s/o -------- Miah Chowdhury of Vill +P.O- Durgapur PS.-------------, Dist.----------- Tripura, aged 27 yrs, by Cast Muslim, by profession- Cultivation, the citizen of India, do hearby solemnly affirm and declare as under:-

 b), Tama Rani Sukla Das, D/o- Tapan ch. Das of  Vill +P.O- Durgapua, PS.Sonamura, Dist. Sepahijala Tripura, aged 27 yrs, by Cast Hindu, by profession-Divorcee Woman, citizen of India, do hearby solemnly affirm and  declare as follows:-

1)             That , we are intending to marry each other and our marriage to be solemnized under the provision of Special marriage Act,1954.

                                                                                This is true to our knowledge

2)    That,we neither have a spouse living,

a)                         That, neither we are incapable of giving our valid consent due to unsoundness of  mind.                                                                                  

b)    That neither we have been subject to recurrent attacks of insanity or   

            epilepsy.

c)     That, we both have completed the age of 27 years .  

        d) That, we are not within the degrees of prohibited relationship.

                                                                  This is true to our my knowledge

    

3)    That, this declaration is made only for the purpose of solemnization of our marriage U/S 5 of Special Marriage Act, 1954.
